I am getting something similar, except that after I click the submit button, it tells me it couldn't find what I was looking for, yet still creates the forum.
I am still unable to create second forum within my account. Here is the error when I try to create new forum "What I LIKE!"

Sure. Just go to http://mylifeorganized.uservoice.com/ as admin and try to create new (second) forum. I got the following error:
"There was a problem with the following field: Key is invalid." Can I create more then 1 forum?
